Service Coordinator - Work at home (must live in or close to service area)

Make a difference every day by working one-on-one with older adults to coordinate services that help them remain in their own homes.
Primary
Responsibilities:
Perform screening & assessment of clients.
Develop a plan of care.
Identify long term services and supports Authorize services, collect, record and data.
Provide follow up and monitoring of client visits.
Help in individuals to remain safely their home.
Required
Qualifications:
Registered Nurse with current Licensure or Bachelor's degree in Social work or related field.
At least 2 years of experience in long-term care gerontology and/or care coordination is required Experience working with MS Word, Excel, and Outlook Ability to travel in assigned region to visit members in their homes - Applicant will need to live close proximity to client service area which includes Adams, Hall and Clay Counties in Nebraska.
Preferred
Qualifications:
Medicaid Waiver experience Case Management and/or Care Coordination experience Behavioral health experience Field based work experience Experience with electronic charting Experience in long-term care, home health, hospice, public health, or assisted living Background in managing populations with complex medical or behavioral needs Knowledge of community resources Our ideal applicant is a good communicator, detail oriented and computer proficient.
