The State of Nebraska, Department of Health and Human Services, Developmental Disabilities Division is looking for creative problem solvers to assess and provide services to Individuals with Intellectual and Developmental Disabilities. The successful candidate is someone who can manage diverse relationships, is action oriented, communicates effectively and is organized. Job duties include: Engaging, developing, coordinating, facilitating, documenting and evaluating the transition process of individuals with developmental disabilities, who may or may not have complex medical or behavioral needs, from an ICF, Nursing Facility, or community setting to the least restrictive environment; application of screening tools and referral for further assessment; facilitation of individual/family meetings; referrals for services; development and coordination of Individual Program Plans, for Person Centered Planning; monitoring provision of services and evaluation of outcome achievements. Frequent regular day and/or occasional overnight travel is required. May work irregular hours to accomplish workload.
Hours: 8-5, Monday - Friday.
Qualifications / Requirements:
REQUIREMENTS: Bachelors degree and professional experience in: education, psychology, social work, sociology, human services, or a related field and experience within a specialized developmental disabilities service system in delivery of habilitation and/or case management services.
PREFERRED: Two years'' experience in services or programs for persons with intellectual or other developmental disabilities and /or two years of experience with focus on physical and/or mental health needs.
OTHER: Valid driver''s license or the ability to provide independent authorized transportation. Applicants that are offered this position will be required to submit to urinalysis to screen for illegal drug use before final appointment to the position.
Knowledge, Skills and Abilities:
Knowledge of: current practices in the field of community-based services for persons with intellectual disabilities and other services for persons with developmental disabilities; person-centered program planning; basic medical terminology; the principles of person centered practices; provision of habilitation services; positive behavioral support techniques; State, Federal, and local resources, statutes and regulations pertaining to delivery of services for individuals with developmental disabilities. Ability to: screen and assess the needs of persons with intellectual or other developmental disabilities; evaluate assessments; develop and assess individual program plans and individual family support plans; adjust services as needed; mobilize resources to meet individual needs; interact and communicate with federal surveyors, other regulatory bodies and others in person via telephone, electronically, and written correspondence to exchange information and to respond to information requests; report non-compliance to appropriate agencies; develop working relationships with individuals with intellectual or developmental disabilities, their families and guardians, interdisciplinary team members, agency representatives, and individuals or advocacy groups; analyze behavioral data; conduct formal assessments; monitor services provided; apply agency and program rules, policies, and procedures; think critically; organize, prioritize, evaluate, and address program/operational data.Estimated Salary: $20 to $28 per hour based on qualifications.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
